A pilot study of the Sensation-Emotion-Cognition (SEC) model found that participants’ confidence in managing their tinnitus more than doubled, while their related emotional distress was nearly halved. What is the Sensation-Emotion-Cognition (SEC) Model?
The SEC model is a clinical framework developed by Danesh and colleagues. It directly targets the three interconnected dimensions that shape the tinnitus experience. The sensation component refers to the perceived sound itself. Emotion encompasses the negative feelings—anxiety, frustration, fear—that the sound can trigger. Cognition involves the thoughts, beliefs, and attention focused on the tinnitus.
“Tinnitus is not typically curable, but it is often manageable with structured, multidisciplinary care,” the authors note. The SEC model is designed to provide that structure. It moves beyond simply trying to mask the sound and instead addresses the emotional and cognitive reactions that often cause the most significant problems. How the SEC Protocol Was Tested
María del Carmen Moleón González, Farzon Danesh, and Ali A. Danesh integrated data from an exploratory retrospective cohort and a prospective study. Their prospective analysis focused on 16 participants who completed the full SEC protocol.
The intervention was multifactorial. All participants received sound therapy and counseling with relaxation training. The cognitive behavioral therapy (CBT) component was delivered flexibly to suit different needs: through unguided, module-based internet CBT; clinician-guided internet CBT; or six traditional, therapist-led CBT sessions. The researchers measured outcomes using two tools: the 4C questionnaire to assess management confidence and the SAD-T scale to gauge emotional distress. They compared scores before and after the intervention.

Concurrently, the emotional distress captured by the SAD-T scale fell sharply, from an average of 4.75 down to 2.38. This reduction was also statistically significant, with a similarly large effect size (Cohen’s *dz* = 0.99). In practical terms, people felt more in control and significantly less troubled by their tinnitus after completing the SEC-based program.
The success of the CBT element in this protocol echoes findings from other fields. For instance, research on CBT for insomnia (CBT-I) shows that structured cognitive-behavioral approaches can produce lasting improvements by changing maladaptive thought patterns and behaviors.
Implications and Cautions for Tinnitus Care
This study offers a tangible, audiology-led pathway for comprehensive tinnitus management. The SEC model gives clinicians a clear template to address each facet of a patient’s distress. By combining sound-based therapy with psychological strategies, it acknowledges that the problem exists not just in the ear, but in the brain’s emotional and attentional networks.
